General: Based on Interview, Staff stated that on 10/13/11 approximately sixty (60) children enrolled in the center went to Huber Farms in Indiana for a field trip. Staff stated further that eleven (11) staff and an unknown number of parent volunteers were also on the trip. Staff stated that children were divided up between the parent volunteers and they were responsible for the care of their assigned children for the duration of the field trip. Staff confirmed that although the parent volunteers had complete supervisory control over the children that were assigned to them, no criminal background checks or child abuse and neglect checks had been conducted. During interview the assistant director stated that she and the director were unaware that criminal background checks and child abuse neglect checks were to be conducted if parent volunteers were going to have complete supervisory control over the children in their care.
