General: Based on observation of the center's recorded video dated 03/26/26, interview and review of documentation, fourteen (14) children and three (3) staff members were present at approximately 10:05 am on 03/26/26 in the preschool room. While two (2) staff members interacted with other children and a third staff member swept the floor, a four-year-old child put his left hand up the bottom, right leg opening of a five-year-old child's shorts at the light table. The five-year-old child reported the incident to the staff member, who swept the floor.
