General: Based on interview, the licensee failed to report to the cabinet a substantiated finding of child neglect for an employee, date of hire 04/24/12. The director stated she received the processed child abuse/neglect check on 05/14/12, but failed to review the document before she filed it in the employee's file. General: Based on observation, a child's medication log was posted in plain view for anyone who entered the Toddler I room. Also, children's allergies and special conditions were kept in a transparent purple folder which was posted in plain view of anyone who enter the Toddler II classroom.
