General: Based on a review of documentation, this center submitted altered/falsified TB documentation, on 06/06/14, for two caregivers hired 09/01/03, a caregiver hired 08/01/09, and a caregiver hired 09/01/98. A review of the four (4) TB documents determined the old dates on the forms were altered to make the TB information current. A review of the Report of Tuberculosis Screening for the two (2) staff members hired 09/01/03 determined the screening was conducted on Saturday, 08/10/13. A review of the Report of Tuberculosis Screening for a caregiver hired 09/01/98 determined the screening was conducted on Sunday, 08/11/13. Interviews with the health facilities confirmed the facilities were not open on Saturday or Sunday. During interview, the director stated she used White-out to cover the dates of the four TB documents so the physician could enter new dates on the old forms.
