General: Based on interview and review of documentation, a five-year-old child was left unsupervised from 7:53 a.m. until he was found between 9:05 a.m. and 9:10 a.m. The child was checked into the opening classroom at 7:31 a.m. The school-age children were transitioned from the classroom to the cafeteria, while the preschool child remained in the classroom, asleep. One (1) staff member stood in the doorway of the classroom and the other staff member stood in the hallway as the school-aged children moved from one location to another. Once both staff members were back in the classroom, the sleeping child was directed to wake up. One (1) staff member signed the child out of the classroom at 7:45 a.m., and instead of walking him to his preschool classroom as usual at 7:53 a.m., she left him in the classroom and went to her next assigned classroom. The second staff member in the classroom believed that the staff member who had signed him out had escorted him to his preschool classroom. The staff member who had signed him out forgot that he was present between signing him out at 7:45 a.m. and leaving the classroom at approximately 7:53 a.m. The child was later found standing in the hallway by a school staff member, outside of the director’s office, near the opening classroom door. The child was alone and visibly upset. The child was taken inside the building to the director and staff who were in the cafeteria eating lunch. Interview with staff stated that the children and staff had returned from the pool at approximately 11:25am and went to use the bathroom and wash up for lunch which takes approximately ten (10) to fifteen (15) minutes. Staff stated that the staff and children then went to the cafeteria for lunch and that the child must have accidentally been left in the bathroom. It was determined that the child was alone for approximately ten (10) to fifteen (15) minutes and during that time was able to get his backpack and exit the building without staff knowledge. Based on interview, the director failed to assure that staff implemented facility procedures. Interview with staff and the director found that there was no written procedure for assuring children are supervised during transition periods. However, a verbal procedure was in place for bathroom times. Staff are to do head counts, check under the stalls for feet and ask if anyone is in the bathroom. On June 11, 2018 a five-year-old child was left unsupervised in the bathroom. The staff responsible for the child reported she asked if anyone was in the bathroom and checked for feet but did not do a head count. The child who was left unsupervised, left the facility and was found by a couple driving by the school. The director stated that the staff who was to be supervising the child was not held responsible but the child was suspended from the program for exiting the building.

It was determined that the child was alone for approximately ten (10) to fifteen (15) minutes. The staff responsible for the child indicated that she went into the bathroom and asked if there was anyone inside and also looked under the stalls to see if she could see feet. When she received no answer and no feet were visible, she left the area. This staff also admitted that the five-year-old who was left unsupervised is known to put his feet up so they cannot be seen. The child exited the restroom, got his backpack and left the building where he was found by passing adults who returned him to the program. After the child's return, his mother was contacted to pick him up as the director suspended him from the program due to his exit. The child was interviewed by a DCBS representative and stated that when he found himself alone, he was afraid he was in trouble so left to find his grandmother who works nearby. The expectation by staff that the five-year-old child is responsible for being unsupervised and expected to know the correct behavior is not developmentally appropriate.
