General: Based on observation and interview, a bottle warmer was stored on the window sill. The cord of the bottle warmer extended from the window sill, along the wall, and was plugged into a power strip. The power strip was plugged into a wall outlet behind the microwave. The microwave was on top of a refrigerator and the refrigerator set on top of a television stand. The cord of the bottle and the power strip were accessible to a child, enrolled 10/24/11, who pulled the bottle warmer cord and caused the hot water in the bottle warmer to fall on the child's face, left arm and shoulder. The microwave and the refrigerator on the televsion stand blocked the child's activity. General Administration 89 - Licensee responsibility 922 KAR 2:110 - Section 2 General. Toddlers': Based on observation and interview, a bottle warmer cord hung down from a window sill that was three (3) feet from the floor on 2/27/12. On 02/27/12, a child pulled the bottle warmer cord which caused the bottle warmer, a bottle to fall to the floor and the hot water spilled on the child. This child received 1st degree burns on the left side of his face and shoulder and 2nd degree burns on his left arm. A testing of the hot water temperature with a calibrated thermometer determined the water temperature registered two hundred (200) degrees Fahrenheit, within two (2) minutes, in a bottle warmer stored stored on a three (3) foot high window sill located in the toddlers' room on 02/28/12. Based on Shriners Burn Institute Time/Temperature relationship in scalds, i.e., a child could or may receive a scald injury in about one (1) second from a water temperature of more than one hundred and fifty-five (150) degrees Fahrenheit.
