General: Based on observation, interview and review of documentation, the center failed to ensure: 1.) Each adult, including parents and guardians at drop-off and pick-up, shall wear a face mask while inside a child care center or family child care home, in accordance with 922 KAR 2:405E Section 8 (1). Upon entrance, two (2) caregivers did not wear a face mask. Also, three (3) caregivers were observed in the outdoor play area without wearing a face mask. A parent did not wear a face mask in the center at drop-off at 9:45 a.m. 2.) Children and adults shall be screened for fever and contagious symptoms upon entry into the child care center or family child care home each day and shall not be allowed to enter if displaying a contagious fever or symptom of COVID-19, in accordance with 922 KAR 2:405E Section 7 (1). A parent and a child were not screened for a fever and contagious symptoms at drop-off at 9:45 a.m. 3.) New staff shall undergo name-based background in accordance with 922 KAR 2:405E Section 11(5). Seven (7) caregivers hired on 7/13/20, 8/4/20, 8/19/20, 8/17/20, 10/8/20, 10/12/20, and 11/12/20, files were not available for review to verify name-based criminal records check and child abuse and neglect check had been submitted and/or processed. Interview determined the staff files had been taken home. 4.) The maximum group size shall be fifteen (15) children per group for children age twenty-four (24) months and older, in accordance with 922 KAR 2:405E Section 4 (1)(b). Twenty- two (22) children, aged one-year-old to five-years-old, were combined together in the outdoor play area. 5.) A child care provider shall stagger playground time between classroom groups so as to separate one group of children from another, in accordance with 922 KAR 2:405E Section 4 (7)(c). A preschool class, a three-year-olds’ class, and a two-year-olds’ class were combined together in the outdoor play area. 6.) Machine washable toys shall not be used, in accordance with 922 KAR 2:405E Section 6 (3). A stuffed Batman toy and a pink stuffed unicorn toy were on a shelf in the three- year-olds’ room. Five (5) large pillows were in a reading corner in the four and five-year-olds’ room.
