General: Based on observation and interview, the following was found: 1) As the surveyor approached the child-care center, a staff member was observed outside the center talking to a non-staff individual. When the surveyor entered the facility, the same staff member greeted her and took her to the adjoining room, Preschool Classroom. A staff member was observed sitting at a table in the Preschool Classroom. A staff member used a hand motion to tell her to go to the Infant Classroom. The Infant Classroom had seven (7) children, ages four (4) months-old to fourteen (14) months-old with one (1) staff member, placing them out of ratio until the other staff member was moved to that classroom. 2) In the Toddler Two (2) classroom, the surveyor observed seven (7) children with one (1) child that appeared younger than the rest. The surveyor asked staff member the age of the child and she replied, "he is one". The rest of the children were age two (2) years-old. This classroom was out of staff-to-child ratio.

General: Based on observation, the following was found accessible to the children: 1.) In the preschool classroom, there was a dirty broom and dust pan. 2.) In the two-year-old classroom, there was a can of glade air freshener sitting on a child sized table. It is important to note that this classroom was not currently in use at the time of the inspection. 3.) In the kitchen, there were numerous containers of bleach, cleaning products and rolls of trash bags sitting out on the floor. In addition, these items were also being stored in cabinets in the kitchen. The kitchen door was unlocked and propped open during the time of inspection. The cabinets also did not have any kind of locking mechanism to prevent children gaining access to these products. 4.) General: Based on review of documentation and interviews, a three-year-old child who reportedly is allergic to strawberries took a drink of strawberry flavored milk while at the day care on 1/15/19 at approximately 12:10 p.m. and experienced a reaction at approximately 12:45 p.m. An interview with the child’s parent found that she and her child walked from the day care center to the head start center, which is approximately a two (2) minute walk. She reported that her child had hives on his face and his tongue was swollen by the time they arrived at the head start center. The parent stated that her child’s head start teacher told her that he could not stay and needed to see a doctor. The parent stated she called an ambulance and she and her child were transported to St. Elizabeth hospital located in Newport, Ky. She stated the doctor gave her child Benadryl and a steroid while at the hospital for an allergic reaction to strawberry milk. An interview with the head start teacher found that she received a note from the child’s parent the next day that confirmed that he was seen in the emergency department and able to return to school. On 1/16/19 during an onsite visit at the day care, the surveyor reviewed the child’s enrollment form and found strawberries listed as a food restriction. The staff person who gave the child the strawberry milk on 1/15/19 stated, “It was just a treat that I did for the kids. I added the strawberry syrup to the kid’s milk. I know he has an allergy, but it did not register in my mind, because it was artificially flavored syrup.” She stated the child took one (1) drink of the milk and then poured it out and said,“This milk is pink. I am not supposed to have strawberry.” The director told the surveyor that she thought the child was only allergic to fresh strawberries because he has consumed strawberry flavored food in the past and has not had any type of reaction. She stated she has not been able to determine what actual food restrictions the child has due to not having clarification from the child’s parent and not having received medical documentation. The facility did not have any documentation for the foam squares or the company that installed the surfacing; therefore, the surveyor was unable to determine if the surface material under the slides provides adequate cushioning for the children during falls.
