General: Based on observation, interview and review of documentation, the following was found: 1) The surveyor found a Kentucky Applicant Registry and Employment Registry (KARES) document that showed a staff member (DOH: 08/03/23) lived in Tennessee in the last five (5) years. The staff member’s file did not contain a completed child abuse/neglect check from Tennessee. The surveyor did not observe the staff person working alone with children. 2) A staff member (DOH: 07/07/23) lived in Mississippi in the last five (5) years. The staff member’s file did not contain a completed criminal records check and child abuse/neglect check from Mississippi. 3) A staff member (DOH: 8/1/22) lived in the state of Illinois in the last five (5) years.
