General: Based on observation, this regulation was not met: 1) Upon walking into the Threes Number Two (2) Classroom, the children were sitting at the table in the classroom alone. The teacher had walked out of the room to the bathroom in the adjoining room to help a child wash their hands. It could not be determined how long she had been in the bathroom prior to the surveyor entering in the classroom. The teacher returned approximately twenty (20) seconds after the surveyor entered into the classroom. 2) The Surveyor also witnessed the teacher in the Fours Number One (1) Classroom leave the classroom at least three (3) separate times to go into the adjoining classroom. It was around lunch time, so the surveyor thought she was getting food from the other classroom, but she did not have anything when she returned. The first time, the teacher was out of the room for approximately thirty (30) seconds. The second time she left the classroom; she was gone for approximately one (1) minute. The third time, it could not be determined how long she had been out of the classroom. She was in the adjoining classroom with a cot in her hands.
