General: Based on interview, upon arrival at the facility it was found that the site supervisor was not present and the other staff person that is normally in charge in the mornings until the site supervisor arrives was out sick. The staff person stated she did not have access to staff files and other required documents. She stated that the site supervisor works before school care and then leaves and comes back at 12:00 p.m. and works until the facility closes. The staff person contacted the site supervisor. General: Based on review of documentation presented at the time of the survey and a review of KARES, it was found that eight (8) staff (Hire dates: 4/12/16, 10/26/18, 8/15/18, 10/14/18, 9/28/17 and 2/14/18) had evidence on file at the facility of having their fingerprints taken. However, there was no evidence or documentation to veirfy these staff have been determined eligible for employment. Only one (1) of these staff was listed under this facility in KARES. General: Based on observation, several blue and red tri-fold vinyl nap mats were observed stored in a cabinet by the door that enters the classroom. The mats were stored in a manner that allowed the mats to touch. An interview with the staff in the room found that the mats are cleaned on Fridays. Further inspection of the room found blankets and pillows stored in a pile inside the cabient under a changing table that is used for storage. An interview with the staff in the room found that the blankets are taken home on Fridays and washed. Due to these itmes not being cleaned until Friday of each week, the manner in which they are stored is unsanitary and allows for cross contamination of germs.

General: Based on interview at the time of the survey, it was found that two (2) children who were present had started attending on 1/14/19. The staff person stated she did not have the children's file that contained all required information. The staff person stated that the site supervisor had the information. The staff person contacted the site supervisor who arrived not long after the surveyor arrived. The staff person stated the site supervisor normally works the before school hours and then leaves and comes back around 12:00 p.m. each day and works until the facility closes at 6:00 p.m. An interview with the site supervisor found that she had the children's files in her car because she needed to make copies of the information to keep on file at the facility before giving the information to their Central Office.
