General: Based on Interview, it was found that on 7/8/14 while on a field trip to the Louisville Zoo an eleven (11) year old child was missing from a group of twenty-four (24) children for approximately five (5) to ten (10) minutes. The group of children had been in the bathroom and before leaving the bathroom all twenty-four (24) children had been accounted for. However, the group moved on to the monkey and gorilla exhibit when staff noticed a child who had been in the group when they left the bathroom was no longer with the group. One (1) staff person went back to the bathroom and did not find the child. The staff person retraced her steps to all of the areas the group had been. Meanwhile the staff person in charge saw a zoo employee on a golf cart and informed him they had a child missing and gave him the child's name and a description of what the child was wearing. The staff person went to look for the child and found her at the main entrance of the zoo. The staff person in charge stated in interview that the children had been instructed to meet at the main entrance of the zoo if they get separated from the group. The child's parent was informed of the incident once the group arrived back at the facility at approximately 6:00 p.m. Staffing Requirements
