Three (3) children's enrollment information (DOE: 07/15/25, 07/23/25, and 07/28/25) did not contain a contact telephone number for their respective preferred hospitals. 2. One (1) child's enrollment information (DOE: 07/29/25) did not contain a preferred physician with contact information, nor a preferred hospital with contact information. In the space provided for preferred hospital, the guardian had written "closest".

General: Based on interview and review of documentation, for the first, second, and third quarter of 2025 there was no evidence of quarterly earthquake, tornado, or lockdown drills having been conducted available for review. Additionally, for the third quarter of 2025 there was no evidence of a quarterly shelter-in-place drill having been conducted available for review. The provider stated she does not do them regularly as the children are uncooperative.
