General: Based on observation, the following was found: 1.) On the playground used by the Head Start classrooms, there were areas on the playground where the black underpinning was exposed. This causes a tripping hazard to the children using the playground. 2.) On the smaller playground used by the early Head Start children, there were weeds that needed to be pulled. As well, there was a lack of grass in a certain area, which resulted in mud and the ground being exposed. General: Based on observation, this regulatory requirement was not met. During a tour of the facility the following was found: 1) In the bathroom located in Room # 6, a portion of the baseboard had come loose from the wall near the toilet. 2) In the bathroom in Room # 7, there was an area near the top of the baseboard where the paint had peeled away from the surface. It was also found that the ventilation fan was not working properly. 3) In the bathoom in Room # 8, it was found that the ventilation fan was not working properly. 4) In the bathroom in Room # 5, it was found there was no ventilation fan. 5) In the hallway where the kitchen is located, there was a large bathroom that contained four (4) toilets and four (4) sinks. One (1) of the toilets was not working. In addition, there was no ventilation fan. The toilet bowl in three (3) of the toilets was stained and dirty. 6) In Room # 4, there was a large bathroom with three (3) toilets and one (1) urinal. This bathroom could also be accessed from the hallway across from the kitchen. There was not a ventilation fan in the bathroom and the baseboard located behind the toilets was pulled away from the wall. In addition, the baseboard had an accumulation of dirt and grime on the surface. 7) In the bathroom in Room # 3, the ventilation fan was not working.
