General: Based on observation, this regulatory requirement was not met. The outdoor play area contained the following items that were in disrepair: 1. A yellow rocking angelfish did not contain handles. 2. A Little Tikes rocking alligator did not contain handles. 3. A small stationary rocking horse contained a broken handle. 4. A green riding tractor did not have a steering wheel. 5. An orange seesaw did not contain handles. 6. A plastic yellow shovel was cracked on the playground. 7. A building located inside the outdoor play area contained a door that was in disrepair. The bottom section of the door was cracked and the surfacing was hanging off. Equipment and toys shall be safe, sound, clean, and in good repair.
