General: Based on interview and review of video footage dated 01/10/24, a staff member hit a child on the face in the preschool room. Interviews determined the child screamed very loudly whenever he did not want to follow instructions. Video footage showed that at approximately 2:06pm on 01/10/24, the child's mouth opened near the staff member's face and the staff member open-handedly slapped the left side of the child's facial cheek. The child immediately cried. The same staff member picked up the child and sat the child in a chair at approximately 2:07pm. The staff member left the room at approximately 2:08pm on 1/10/24. Interviews determined that several days prior to the incident, the same staff member told the child that if he did not stop screaming then she would push her fist down the child's throat. Another caregiver heard the threat but did not report the incident. In addition, in 2023 (exact date unknown), a staff member pulled a child down the hall because the child ran and refused to follow a staff members directive.
