Further review of staff files revealed written documentation on the CAN check that a third staff member hire date 8/22/16 had lived in Tennessee in the last five (5) years; however there was not an out of state criminal record check for Tennessee on file at the time of the survey. Interview with the director revealed the third staff member had not worked alone with children. The director also stated she had completed the out of state background check; however, she did not present it at the time of the survey.
