General: Based on observation during the inspection, the following was found: Ladybug Room 1) There were two (2) electrical outlets that were not covered. After school Room 1) There was one (1) electrical outlet that was not covered in the bathroom adjacent to the handwashing sink. Busy Bee Room 1) There was a light switch that was missing the face plate; the inside wires were exposed. There was a crib located under the light switch, which made it accessible to a child. Fireflies’ Room 1)There was a soiled mop and a toilet brush adjacent to the toilet located in the bathroom to the left within the classroom. Review of Division of Regulated Child Care (DRCC) records revealed a regulatory violation was identified during the renewal inspection conducted on 02/22/24 due to a light switch that was missing the faceplate with the inside wires visible in the Busy Bees room. There was a crib located by the light switch, which made it accessible to a child. Further review revealed the approved plan of correction submitted to DRCC by the licensee stated, "The Director purchases a new face plate and put electrical cover in the new one and the put covers in the other outlets in the Busy Bees, ones, twos threes, strawberry land and the school age room Director replace light switch in the twos and threes room. The date of correction was documented as 03/09/24. The approved plan also included the following action taken to assure ongoing compliance: "The Director will make sure monthly that all light switch and electrical outlets are cover and working properly". Upon the Division of Regulated Child Care (DRCC) representative's entrance into the Infant room, one (1) of the staff members quickly exited the room and entered an adjoining room where six (6) toddlers were sleeping on mats without a staff member in the room. Not all of the sleeping toddlers were within the scope of vision and range of voice of the staff members who were in the Infant classroom. The amount of time during which the toddlers were not provided with adequate supervision is unknown. During a subsequent visit to the center on 04/29/2022, two (2) staff members were present in the Infant classroom with seven (7) infants and toddlers. There were five (5) one-year- old toddlers present in the adjoining classroom with no adult present in the room. Four (4) of the one-year-old toddlers were sleeping on cots and one (1) was in a crib. One (1) of the staff members in the Infant room claimed responsibility for the toddlers upon being asked who was supervising the sleeping toddlers. Not all of the sleeping toddlers were within the scope of vision and range of voice of the staff members in the Infant room as they were moving about the room caring for other children. Observation in the lobby area revealed a can of black spray paint, a can of yellow spray paint, several paint brushes soaking in soiled water, a bottle of Fantastik-Bleach All Purpose Cleaner, a three (3) gallon container of paint, and a pressure washer, all accessible to children. The chemicals contained warning labels of: Keep Out of the Reach of Children. Observation in the kitchen revealed 2 containers of Clorox Fraganzia, a container of Lime Away, a foaming disinfectant, one can of Krylon spray paint, a gallon container of paint, and a container of Power clean. The aforementioned items were found in unlocked cabinets in the kitchen, with doors which were propped open, allowing accessibility to the products, by the children.
