Observation of the child care center entrance ways to the Toddler and Infant classrooms revealed gates used to prevent access to outside areas by the children in each classroom. The gates installed were not designed or positioned to guard against entrapment or situations that could cause strangulation. In addition, the white wooden fence, which lead into the Infant classroom, contained roughly edged and splintered surfacing, with large gaps between each connected piece of wood. The wooden gate which lead into the Toddler classroom was approximately six (6) to seven (7) inches above the floor.
