General: Based on observation, interview and review of documentation, the child care center director failed to assure compliance with regulatory requirements regarding the health, safety and comfort of children. Interviews with staff revealed that all infants in the Infant room are swaddled or sacked until one (1) year old when they move from the room. Further interviews revealed that not all of the babies were soothed by the swaddling. A review of written documentation and interview revealed that parents repeatedly requested that a 10 month old child not be swaddled, citing child development recommendations and safety hazards, to stop swaddling at approximately 3-4 months of age. Interviews also revealed the possible safety hazards of a child rolling onto his back and not being able to roll back onto his stomach. The child continued to be swaddled despite the parent's plea to end the swaddling practice. Interviews also revealed that swaddlers were not used according manufacturer's intent. The some swaddlers contained holes for the child's arms; however, the child's arms were not placed in the holes.
