General: Based on review of documentation, the personnel file for a staff member (DOH: 01/09/25) contained a TB test that was expired, it was dated 01/20/23. There was not a written, current statement from a health care provider stating that the staff member was free of active TB or negative results from a current Tuberculin test. The staff in charge stated that he/she was aware that the TB test had expired.

General: Based on observation, review of documentation, and interview, during the inspection the following was found: The personnel file for a staff member (DOH:01/09/25) did not contain verification that the child care center had initiated the process to obtain a completed background check with an eligible employment status on the day of the inspection. Review of the Kentucky Applicant Registry and Employment Screening (KARES) system did not list the mentioned staff member as being employed at the child care center. A background check was presented for review. However, the background check was from a different child care center. A review of KARES verified this information.

Interview with the staff in charge stated that he/she had people ready to hire, but staff files were not available for review at the time of the inspection.
