General: Based on observation, the outdoor play area was not free of unavoidable danger or risks due to the following observations: 1. A metal spike was protruding from the ground near the garage. 2. Another metal spike was protruding from the ground on the left side of the front steps. 3. The chain link fence was not stable, The poles of the fence were positioned in cinder blocks and were leaning toward the ground. The chain link fencing material was drooping toward the ground also. In some areas, the fence was lying so low that it was nearly parallel to ground. 4. A discarded car battery was in the flowerbed located to the right of the front entryway steps. General: Based on observation, a standard-size trampoline was present in the back outdoor play area. There was no barrier around the trampoline preventing access to the equipment. Interview confirmed that care will be provided to toddlers, preschool, and school-age children. Due to the height of the trampoline, older children would be able to gain access to the equipment.

General: Based on observation, the sole crib in the home failed to meet the Consumer Product Safety Commission standards established in 16 C.F.R. 1219-1220. The crib contained a drop-down side. Such cribs have been recalled based on the established consumer product safety standards. Additionally, the side of the crib that did not contain the drop-down rail was no longer secured to the frame. A screw was protruding from the rail and was no longer attached, causing the side of the crib to pull away from the frame.
