General: Based on observation and review of documentation, the health, safety, and comfort of children in care were jeopardized in the following occurrences. 1. A toddler received an injury to her arm from an unknown source while she was in the care of Talk Childcare To Me L.L.C., L383339. Review of video documentation from various cameras to account for duration of the child's presence at the center, revealed the child displayed no indication of injury or loss of use of her right arm until she was placed on a changing table for a diaper change. The top of the changing table was not visible from the camera from which the video was captured. Neither the child nor the staff member's interaction with the child, could be seen in the video. Upon the child being returned to the floor from the changing table, the child's arm movement appeared to be limited. The child was then placed at the table and provided lunch and the child failed to use her right arm while handling her food. The child later received medical attention that resulted in her arm being placed in a cast. 2. The same staff member was observed in video grabbing another toddler by the arm and lifting the child off the ground in a rough manner, leaving the child's legs dangling as the child was transitioned to a chair. 3. A different staff member was observed in video using a cloth to wipe a child's nose, then using the same cloth to wipe another child's face. 4. A toddler climbed on top of the changing table and leaned over the side of the table, resulting in the child precariously hanging from the table with his/her head toward the ground while the staff member was across the room and not in close proximity to the child.
