Interview with the staff revealed that the child had been in time-out and was seated in the hallway chair. Interview with the staff revealed that the child had returned to time-out in the hallway; out of the scope of vision of staff. Observation revealed that the staff person in the Infant Room realized that the child had returned to the time-out chair after a few moments and directed him back into his classroom. At approximately 11:00 a.m., observations revealed a four-year-old child enter the child care center from the outdoor play area and proceed down the hall alone and unsupervised into the classroom.
