General: Based on observation in the School Age room, a collection of blankets was stored on top of the stack of cots. Interview with the staff in the room confirmed that the blankets were used by children throughout the week and will be sent home at the end of the week. The used blankets were not stored in a manner to prevent the spread of germs. In the Younger Twos C room (downstairs), a wooden dry erase easel was precariously sitting on top of a wooden shelf. General: Based on Observation, record review and interview, the child care center director failed to assure compliance with regulatory requirements. During a tour of the child care center on 11/13/15, it was observed that children participated in Computer Wiz Kids classes. This activity required the presence of an individual who, upon review of child care center records, was not documented to have completed a criminal records background or Child Abuse and Neglect Check or to have received sufficiently documented parental consent. Further record review revealed these same individuals werenot authorized by the parents to check the children in or out of the facility. Interview with the administrator of the Computer Wiz Kids program revealed every Friday she takes the participating children from their classrooms to an area alone without child care center staff involvement. She stated the parents paid her for the computer instruction given to the children at the child care center. Interview with the child care center director revealed these individuals were left alone with the children who participated. The director further stated no child care center staff was present when the children participated. The child care center director also stated that the children did not leave the facility. Therefore, he/she did not require that the children sign out or sign in upon return from the activity. The director concluded that he/she considered the child care center responsible for the children when the children participated in the computer class. Upon review of the signed parental consent form for the Computer Wiz Kids activity, the director agreed that the consent forms were not sufficient as to allow child participation without child care center staff present. Further review of the consent document revealed the only authorization granted by the parents for those children who participated was permission to be photographed for advertising or promotional purposes only. General: Based on Observation, record review and interview, the child care center failed to maintain employee records in accordance with regulatory requirements. During a tour of the child care center on 11/13/15, it was observed that children participated in Computer Wiz Kids classes. This activity required the presence of an individual who, upon review of child care center records, was not documented to have completed a criminal records background or Child Abuse and Neglect Check or to have received sufficiently documented parental consent. Further record review revealed this same individuals was not authorized by the parents to check the children in or out of the facility. Interview with the administrator of the Computer Wiz Kids program revealed every Friday she took the participating children from their classrooms to an area alone without child care center staff involvement. She stated the parents paid her for the computer instruction given to the children at the child care center. Interview with the child care center director revealed these individuals were left alone with the children who participated. The director further stated no child care center staff was present when the children participated. The child care center director also stated that the children did not leave the facility. Therefore, he/she did not require that the children sign out or sign in upon return from the activity. The director concluded that he/she considered the child care center responsible for the children when the children participated in the computer class. Upon review of the signed parental consent form for the Computer Wiz Kids activity, the director agreed that the consent forms were not sufficient as to allow child participation without child care center staff present. Further review of the consent document revealed the only authorization granted by the parents for those children who participated was permission to be photographed for advertising or promotional purposes only. Additionally, record review of employee records revealed no documented evidence of verified or applied for Criminal Abuse and Neglect Check (CAN) for employee with a hire date of 11/9/15.
