General: Based on Observation, the child care center failed to maintain equipment in accordance with regulatory requirements. Based on observation, on the playground, four (4) wooden benches approximately six (6) foot long, contained splintering pieces of wood, and nails which were not flush with the wood, in areas in which a child would sit. Additionally, the large metal climbing structure on the playground, contained numerous brackets and joints which contained rust and corrosion, resulting in peeling paint in areas accessible to the children.
