Observation of the floor outside of the bathrooms (in the back of the building) revealed that there were six tiles missing off the floor near the entrance to the bathrooms. Observation of the walls of the men’s bathroom (in the front of the building) revealed that the wall next to the urinal had peeling paint that was accessible to the children. Observation of the ceiling in the women’s bathroom (in the front of the building) revealed that there were water spots and there was a large hole near the light on the ceiling.
