Observation of the upstairs classroom at 2:08pm revealed ten (10) children present ages three-years-old to four-years-old. Interview with staff (DOH: 8/2008) revealed she has eleven (11) children and that one (1) four-year- old child was in the bathroom. The surveyor then walked down the hallway and into a bathroom, in which one (1) of the doors to the toilet stall was closed. Observation of where the bathroom is located revealed it is not within scope of vision and range of voice of the classroom. The surveyor returned to the aforementioned classroom and a child entered the classroom at 2:12pm and the staff member stated that is the child whom was in the bathroom. The aforementioned four-year-old child was observed to be unsupervised for four (4) minutes. Exit interview with the director revealed that the facility was short-staffed during surveyor's visit and was not able to have two (2) staff members present in the aforementioned classroom.
