Children's Records 96 - Enrollment Information 922 KAR 2:110 - Section 3 The following records shall be maintained at the child-care center for 5 years: (1) Sufficient records to: (a) Identify each child enrolled in the child-care center; and (b) Enable the person in charge to contact each child's: 1. Parent at: a. Home; or b. Place of employment; and 2. Family physician; and (c) Identify the name of each person designated in writing by the parent to pick up the child; 1) Identification Information Not In Compliance Findings: General: Based on review of documentation, there was no completed enrollment form that contained identification information for a child in care. During interview the staff stated information was kept on a computer program designed specifically for child care centers. During interview the staff stated information was kept on a computer program designed specifically for child care centers. Review of the information stored on the computer database for this child revealed there was no medical history available on this system.
