General: Based on observation of the Bright Stars Room, a staff member washed bottles at a sink with his/her back turned to the children. A radio on the counter of the sink played loud music. The surveyor entered the classroom, unnoticed by the staff member. The surveyor attempted to gain the staff member's attention several times by speaking to her in a loud voice, but the staff member continued to wash bottles and failed to turn around. During this time, a small child played on an adult size rocking chair, placing his/her head on, but not through, the gap between the seat and back rest of the chair. This was unnoticed by the staff member, as he/she continued to wash bottles with his/her back turned to the children. The surveyor moved in very close proximity of the staff member and used a loud voice to gain his/her attention. The staff member acknowledged the surveyor at that time. The surveyor addressed the concern of the children's safety being compromised, due to the staff member's apparent inability to hear or see them while she washed the bottles. General: While waiting to gain entry into the child care center on 10/11/2021, a little girl was observed in the hallway at the entrance of the center alone. The aforementioned child played with the dog in the hallway for several minutes before entering the bathroom, located two (2) doors down from the office. Continued observation while waiting to gain entry into the center revealed several little boys walking into the hallway from a room, alone, and standing in front of the aquarium located in the hallway. Interview with the staff member in charge revealed that all the aforementioned children were three-years-old. Observations revealed that none of the aforementioned children was within the range of voice or scope of vision of any of the staff members present in the center at the time of the visit. Observations revealed that the three-year-old little girl was four (4) rooms down from her classroom and the little boys were several feet from their class, which was in the cafeteria at the time of the incident. While continuing to wait to gain entry into the center, observations revealed a staff member walking from the cafeteria with their children across the hallway into their room. The same staff member was observed several minutes later, from the front door of the center leaving their room and going back across the hallway to the cafeteria, causing the children in their room to be unsupervised. Observation of the room and interview with the staff member working in the room after entering the center revealed that the aforementioned staff member was caring for twelve (12) children ranging in age from two-years-old through three-years-old.

General: Based on interview, on 06/14/13, a seven-year-old child suffered a sunburn on his back after spending the day outside on a field trip which required medical attention. Based on interview, the child had sunscreen applied to his exposed skin prior to leaving for a field trip, however, the child took off his shirt during the field trip. Interview revealed that no staff person intervened to assure that sunscreen was applied to the newly exposed area of the child's back and said child was not instructed by staff to put his shirt back on as a protective cover against sun exposure.
