At 10:24am the aforementioned staff member called for one of the children who was left behind but the two (2) children kept playing. At 10:25 the staff member of the 2-year-old room walked out of the bathroom with seven (7) children. The staff member then escorted the two (2) 1-year-olds to their classroom thus leaving seven (7) 2-year-olds out of scope of vision for about 20 seconds. The standards established in 922 KAR 2:410e Section 6(3) prohibits the use of machine washable toys. Observation of the four-year-old room revealed there was a child size dress up princess dress accessible for the children. In accordance with the standards established in 922 KAR 2 : 410e Section 4(3), each child shall remain in the same group throughout the day without interacting with another group, expect that sibling groups may be combined if they are the only children in the facility. Observation of the three-year-old-room at 9:05 a.m. revealed there were fourteen (14) children ranging in ages from two-years-old through ten-years-of age. While the surveyor was conducting the inspection and observed there were six (6) children being moved from the three- year-old room to the two-year-old room. Based on a review of documentation,staff members with hire dates of: 01/09/16 and 07/31/15 failed to have completed results from an out of state CRC. A small, multi-colored climbing cube had split pieces of plastic on the area where children sit before sliding down the slide.
