Based on observation in the Two Year old room, approximately 8-10 inches inside an exit door, was a tear in the linoleum which created a tripping hazard. Based on the interview with the staff in charge, she stated that particular door is used infrequently but is used by the two year olds when entering and exiting to the playground. Based on further observation, approximately three (3) inches of rotted blue wood was observed close to the floor, in the door frame, which also contained peeling paint.

Additionally, a large yellow rocking fish failed to contain a handle in which to hold while rocking. A small, multi-colored climbing cube had split pieces of plastic on the area where children sit before sliding down the slide.
