Additionally, during nap time, school age children, up to the age of eleven-years-old slept on cots that failed to be sufficiently sized for school-age children; 2) A seven-month-old infant was observed to sleep face down on the floor, with a bib around her neck. The same infant was later observed sleeping in a crib with a bib around her neck; 3) Multiple electrical outlets throughout the center failed to be covered; 4) A phone charger was plugged into wall in the Super Star room. The cord was stretched from the wall and laying on floor, not in use; 5)A bottle warmer was sitting on top of a small refrigerator, while plugged in and in use, in the infant room. One (1) mobile infant was present; 6) An electric radio tha was in use and plugged into an electical socket while sitting on the narrow surface of the frame of the sink in the toddler room; 7) The kitchen door remained open for the duration of the inspection, leaving the stove and cleaning supplies accessible to children; and 8) A Bumbo infant seat was observed in the infant room. The Bumbo seat failed to be equipped with straps that are required per the manufacturer's recall of the equipment.
