General: Observation of the carpet in room 4A revealed it was unraveling, leaving a long piece of woven material causing a potential tripping hazard. Observation of the outside playground revealed three (3) concrete splash blocks located under the water spouts and attached to the child care center were broken, leaving the concrete with sharp pointy edges. In interview, the director stated the children play in the area where the broken concrete splash blocks were. Observation of the outside playground revealed six (6) concrete blocks that were on the outside perimeter of the wooden picnic table. The concrete blocks were a potential tripping hazard. In interview, the director stated the picnic table legs were supposed to be set on the blocks so the table would not sink into the mulch. Observation of a wooden structure revealed the red paint was chipping off in various areas of the structure; there was a piece of wood securing the slide that was rotted and splintery. Further observation of the wooden climbing structure revealed a yellow climbing chain with four (4) wooden planks dividing the chains that were rotted and the wood was splintery on the outside edges.

Based on interview, on 10-15-13 at about 9:50 a.m., a child was struck in the head with a cot as a staff person was putting the cots out for nap time. As the staff person was carrying the cot past the children watching a television program, two children were grabbing at her feet which caused her to loose her balance and the hard plastic corner edge of the cot struck a child on the back edge of her head resulting in the child requiring two staples to her head as applied by a health professional.
