Review of video documentation revealed on 2/5/19 a staff member was hitting a female child, who was in a crib, on the bottom; sometimes the staff member missed the child’s bottom and hit the child on the back. The staff member continued to hit the child over and over again. The staff member was then excessively rocking the crib side to side, making the child roll back and forth. The staff member stopped rocking the crib, grabbed the child very roughly by the arm and leg and stretch them out, then went back to rocking the crib very excessively; this happened on at least five (5) occasions. The staff member stopped rocking the crib, began to hit the child on the bottom again, then went back to excessively rocking the crib. This incident lasted for more than three (3) minutes. Interview with staff members revealed even after viewing the documentation of the incident on 2/5/19 no one went to check on the child in the crib to make sure she was okay. Continued interviews revealed the child care center did not disclose to the parents of the child in the crib about the incident until 2/11/19. Review of documentation revealed the staff member involved in the incident was suspended on 2/5/19 and returned to work on 2/7/19. Observation the refrigerator unit located in the Infant “A” classroom revealed the temperature gauge read 49 degrees. Continued observation in Infant room “C” revealed the temperature gauge read 50 degrees Therefore, the refrigerator was not maintained at the required temperature reading of forty (40) degrees Fahrenheit or below. Observation of the refrigerator located in the Infant B classroom revealed ten (10) bottles that were stored with an indicating thermometer measuring fifty (50) degrees Fahrenheit at the time of the survey. Continued observation revealed a refrigerator located in Room 24 which held eleven (11) bottles and had an indicating thermometer that measured forty-eight (48) degrees Fahrenheit at the time of the survey.
