Based on interviews and review of documentation, the child care center director failed to assure compliance with regulatory requirements regarding the health, safety and comfort of children in care. The information obtained from DCBS intake notes and interviews revealed incidents occurred involving a five-year-old child exposing his genitalia to other children and inappropriately touching another child's genital area.

Observation in the Toddler classroom revealed that sippy cups containing water were located in the children’s cubbies. Interview with staff in charge revealed that the children receive water in said sippy cups during AM snack. Continue interview revealed that said sippy cups are kept in the classroom to be reused when the children return from outside, therefore are used twice before being washed.
