General: Based on observation, interview and review of documentation, the center failed to comply with the regulatory requirements specific to the current pandemic, as established in 922 KAR 2:405E. Upon arrival into the licensed center, center staff did not screen the two (2) surveyors for fever in accordance with 922 KAR 2:405E Section 7 (1). Machine washable toys were not removed, as required by 922 KAR 2:405E Section 6 (3). A large bucket of stuffed animals and an assortment of doll clothes were observed in the four-year- old classroom. The center did not keep children in the same group throughout the day, without interacting with another group, as required in 922 KAR 2:405E Section 4 (3). Based on interview and observation, children from two (2) separate groups are combined at the begining of each day for early arrival, and again at the end of the day for aftercare. In accordance with 922 KAR 2:405E Section 8 (4), a child who is five (5) years old or younger shall not wear a face mask; however, a five-year-old child was observed wearing a face mask. Interview with the staff in charge confirmed that the aforementioned child regularly wears a face mask. Individual bedding failed to be contained in a manner to prevent the bedding belonging to different children from touching one another. The bedding stored in the three-year-old classroom was stored in wooden cubbies. The bedding expanded outside of the cubbies and was touching the bedding from other cubbies. 922 KAR 2:405E Section 6 (5) requires bedding to be separated and stored in individual labeled bins without touching another child's bedding. In accordance with 922 KAR 2:405E Section 4 (9), a child care center shall not provide access to vistors. However, observation revealed the school's Computer/Spanish teacher visited the preschool classroom and provided materials and oversight for a computer activity. Interview with the staff in charge confirmed that the visiting teacher was not a staff member of the licensed center. Additionally, several adults were observed in the community area, located outside of the three-year-old classroom and was later identified as the school age area. The adults were church members who were participating in a bible study and other church functions. Additionally, several of the adults were not wearing face masks.
