Observation of the wall located in the pre-school classroom revealed a quarter inch hole above the vent near the windows. The hole in the wall was exposed with rough edges. Interview with the director revealed she was in the process of addressing.

General: Based on Review of Documentation, and interview with the director, the child care center failed to maintain written documentation in accordance with regulatory requirements. Review of the daily sign in sheet for the week of November 10-14, 2014 for the after-school program revealed the child care center had not documented the daily attendance for the arrival and departure time of each child. Interview with the director revealed the parents did sign the child out for the day, however there was no sign-in time recorded for each child.

Interview with staff in charge revealed the regulation regarding washing hands after eating was not known to her.

General: Based on Observation, there were two children in the six and seven year old group who did not receive two components during snack service, thereby not meeting the snack requirements. The children were given a grain choice (oatmeal cakes or Moon Pies) and milk or juice. One child was observed being served a Moon Pie(enriched flour) and a cup of water. Another child with allergy restrictions (posted) was served only Sun Chips(whole grain) and no drink. Interview with a staff member revealed she thought that when a component was offered to the child, the regulation had been met.
