General: Based on observation, interview and review of documentation, a bottle warmer within the Crawler's classroom was stored on top of a low mini refrigerator where it was placed on top of a towel. A mobile twelve (12) month-old child was able to reach up and pull the towel, causing the bottle warmer to fall and strike the child on the head, causing a burn due to the device's hot surface temperature. It was reported that a burn mark was left on the child's forehead and face which blistered, prompting the child's parents to seek medical attention.

General: Based on observation, the child care center failed to assure compliance with the requirements specific to the current pandemic as established in 922 KAR 2:410e. Section 8 (6) states staff shall wear gloves when preparing meals and serving bottles. Gloves shall be changed between each bottle feeding. While observing in room E205 the staff member was preparing and serving morning snack and was not wearing gloves. The standards established in 922 KAR 2:410e Section 6(3) prohibits the use of machine washable toys. Observation of room E205 revealed there was one (1) red cloth hat in the kitchen area and there were two (2) cloth pot holders in a plastic bin located in the kitchen area. Section 6 (b) states bedding shall be seperated and stored in individual labeled bins without touching another child’s bedding. Observation of the children's cots that were stacked on top of one another located in rooms E209, E211, E212 and E210 revealed there were children's sheets attached to the cots and the children's blankets were lying on top of each individual sheet. Some of the children's blankets were touching other children's bedding. Therefore, the children's items were not individually labeled, and stored in a bin to prevent the items from touching.
