Upon arrival at the child care center at 10:15am, observation revealed a qualified staff member exited out of the back door of the infant building into the back parking lot and then proceeded to enter into the back door of the school age building. Upon entry into the school age building, it was revealed that the aforementioned qualified staff member was the sole caregiver in the four and five year old room for fifteen (15) children in the age range from four-years-old to five-years-old. Therefore making the four and five year old room to be previously unsupervised for an undetermined amount of time. Observation revealed at 12:15pm, a staff member entered into the kitchen from the back door the staff member then exited back out of back door of the kitchen into the parking lot and proceeded into the school age building. Upon entry, observation in the school age room revealed the aforementioned staff member was the sole caregiver for eighteen (18) children from the age range of six-years-old to twelve-years-old. At 10:00am, a staff member brought the child to her and stated the child was limping, at which time the director contacted the mother. Therefore, it is determined that the injury occurred while the child was in the care of the center.

Neither the owner nor staff member left the cafeteria to retrieve the child or called to the child to return to the cafeteria. Then a short time later, a second child left the cafeteria, entering into the four-year-old room, but the owner acknowledged the second child had left the room by calling the second child back into the cafeteria. Among the documentation submitted for review was an expired Child Care Associate and a high school diploma for the newly identified director. The diploma appeared to be altered as the background of the name of the graduate was much darker than the rest of the document and appeared to be added to the document, covering the original name. Oral and written verification was provided by a representative of the Board of Education referenced on the diploma confirming that the document was altered and the name on the diploma was not the name of a gradaute. Upon entering the child care center on 6/6/16 two (2) adults were observed standing in the foyer. One (1) of the adults entered a room identified as the older two’s / younger three’s where the surveyor followed. At which time it was observed there were twenty-four (24) children in care and no other adult in the room. The second adult from the foyer entered the room. The two adults identified themselves as staff members. Therefore children in the older two’s/younger three’s room were out of the scope of vision for the two (2) staff members standing in the foyer for an undetermined amount of time. Observation on 6/8/16 at 9:15 a.m. revealed that a one-year-old female child had gotten out of the toddler gated programming area and was observed alone walking in the walkway area outside of the toddler and infant programming areas. Surveyor observed the female toddler, unattended in the walkway area for at least one minute prior to the assigned toddler staff, with a hire date of 7/28/14, discovering the female child on the other side of the gated programming area. The staff with the hire date of 7/28/14 was overheard saying, “Get over here girl,” as she retrieved the child. Observation on 6/8/16 at 9:44 a.m. revealed a male child leave the two-year-old room and entered into an adjacent room that was identified as the room used for “summer youth camp” by the owner, out of the scope of vision of the two-year-old staff member. Continued observation revealed a staff member saying “Go get him” at which time a female child entered the room used for summer youth camp and retrieved the male child at 9:46 a.m., escorting him back to the two-year-old room. Interview with a staff member revealed both children were two-year-olds. Observation on 6/8/16 at 9:50 a.m. revealed three (3) children entered into an adjacent room that was identified as the room used for “summer youth camp” by the owner, out of the scope of vision of the staff member assigned to supervise the two-year-old staff children. The children then returned to the two-year-old room. Observation on 6/8/16 at 10:30 a.m. of the room used for summer youth camp revealed a child entered the room and went to the bathroom located on the far side out of the scope of the staff member’s vision. A staff member is heard saying “Just get the toilet paper and come on”; the child exited the bathroom area and returned to the classroom. Observation on 6/6/16 of the seven-year-old and up group on the playground revealed forty-seven (47) children in care with three (3) adults, who were identified as “youth camp” employees, but were observed to have supervisory authority over preschool age children, who are enrolled in the child care center. Interview with one of the aforementioned adults revealed the youngest age child in the group was seven-year-old. Therefore the group was over the maximum group size. Upon arrival at the child care center on 6/8/16 at 8:49 a.m. a staff person with the hire date of 3/7/16 was observed walking on the concrete walkway in front of the building. The aforementioned staff person was observed to pick up her pace when she observed the state surveyors in the parking lot. The staff person with the hire date 3/7/16 entered the Toddler programming area (room 3) simultaneously with the surveyor. At that time the surveyor observed fourteen (14) children in care with one staff person with the hire date 7/28/16. The staff person with the hire date 7/28/16 stated during interview that the second staff person with the hire date 3/7/16 had just gone to one of the other buildings of the child care center to help out for a minute and had returned to help her with care. Continued interview with the staff person revealed the youngest age child was a toddler. Observation on 6/8/16 of the infant programming area at 8:55 a.m. revealed six (6) infants in care with one staff person. Interview with the infant staff revealed that the age of the youngest child was less than one year of age. Observation on 6/8/16 of the infant room at 9:15 a.m. revealed that while the infant staff was involved in performing a diaper change, a female infant crawled on top of a male infant located in a low swing. The female infant crawled around on top of the male infant for approximately twenty (20) seconds and then crawled back down to the floor. The female infant then crawled over to a female infant located in a low swing. Continued observation revealed that the female infant crawled up on the low swing containing the female infant and she took the pacifier out of the infant’s mouth and put the pacifier in her own mouth. The female infant sucked on the pacifier for several seconds and then put it down and proceeded to return to the floor where she continued to crawl around. The infant staff acknowledged that she was not aware that the incidents occurred while she was performing a diaper change with another infant. On 6/6/16 in the older two’s / younger three’s room, five-and-six-year-old group and the seven-year-old and up group the surveyor requested the name of children in care, however the staff was unable to identify the children that were requested at the time of the survey. On 6/8/16 the surveyor requested seven (7) children’s files from the staff designated as the person with knowledge as to where each child’s file was located. The staff in charge of locating the children’s files was still looking for five of the seven children’s files after one hour and thirty minutes. Ultimately the staff was only able to present six (6) out of the seven (7) files requested; therefore if an emergency occurred for any of the children in the child care center, the staff would be unable to quickly retrieve emergency contact information, i.e. parent(s) contact information, and parents preferred hospital, child’s physician information and authorization for medical treatment.
