General: Based on observation during the inspection, the following was found: Building #1 PreK 1 Room 1. There was a piece of wood missing from a low windowsill causing sharp splintered edges. Bathroom off PreK 1 Room 1. The low windowsill was chipping white paint. Building #2 School Age 1 1. The wooden one half (1/2) door did not close securely. The latch of the doorknob did not connect to the strike plate. 2. There was an area of the baseboard that was missing leaving sharp pointed edges. School Age 2 1. The baseboard throughout the classroom was soiled with dust and debris. 2. The hand-washing sink vanity was soiled with dust, debris and unknown substances. 3. The handwashing sink vanity contained a hole causing sharp pointed edges and the wooden molding not to be secured to the vanity. Building #3 Entry Door 1. The inside of the entry door above the doorknob was missing a piece of wood causing an area to contain sharp pointed edges.

General Requirements. Upon arrival at the center; or b. Within thirty (30) minutes of arrival for school-age children; 2. Before and after eating or handling food; 3. After toileting or diaper change; 4. After handling animals; 5. After touching an item or an area of the body soiled with body fluids or wastes; and 6. Upon arrival at the center; 2. After toileting or assisting a child in toileting; 3. Before and after diapering each child; 4. After wiping or blowing a child’s or own nose; 5. After handling animals; 6. After caring for a sick child; 7. Before and after feeding a child or eating; 8. Before dispensing medication; 9. After smoking or vaping; and 10. If possible, before administering first aid; and (d) Use hand sanitizer or hand-sanitizing wipes if liquid soap and warm running water are not available in accordance with paragraph (c) of this subsection. The staff shall wash the staff's hands as soon as practicable once liquid soap and warm running water are available. 710 - Staff Communicable Disease Discussed 922 KAR 2:120. Section 3. General Requirements. (6) A staff person suspected of being infected with a communicable disease shall: (a) Not perform duties that may allow for the transmission of the disease until the infectious condition can no longer be transmitted; and (b) Provide a statement of fitness to return to work from a health professional, if requested.

General: Based on observation during the inspection, the following was found: Outdoor Play Area 1. There was rotten, splintered wood on the side of building #1 where the vinyl siding was missing. 2. There were numerous holes on the side of building #1 in the vinyl, causing sharp, pointed edges. 3. There was a deep hole on the right side of the step of the back porch of building #1. 4. The vinyl siding on the right side of building #3 contained numerous large areas with holes causing sharp jagged edges.

General: Based on observation, during the inspection the following were found: Outdoor Play Area Fence 1. The fence was missing two (2) wooden planks. 2. There was a one (1) inch nail protruding out of the fence. 3. The gate leading into the outdoor play area contained a padlock that was locked; therefore, the gate did not close securely. 4. There was approximately two (2) feet of fence that was missing on the right side of the playground. The staff in charge acknowledged the findings.
