Further review of video documentation revealed that at 4:23 p.m. a four year old child was alone in the classroom for approximately one minute and then at 4:45 p.m. the same four year old left the classroom unsupervised and returned to the classroom at 4:46 p.m. Video documentation revealed that staff member hired on 1-27-14 left the classroom to clean the bathrooms for a total of approximately six minutes, leaving one school age child and two four year old children unattended in the classroom, at one point during this six minutes the school age child left the classroom for one minute leaving the two four year old children. Continued review of video documentation revealed that the school age children were observed to be pulling the arms, dragging, pushing, picking up, carrying forcing them to sit, taking their toys away and running around the classroom chasing one and two year old children and slapping the hand of a two year old child with no intervention by staff member hired on 1-27-14.
