Based on interviews, a child sustained a head injury resulting in blood on his head, collar of the back of his shirt, hands and shoes. Further interviews revealed both staff members in the room failed to witness how the injury was caused. When the injury was discovered, it failed to be reported to the parent or to management. Further audible observation revealed staff with a hire date of 08/19/13 to use a very aggressive and intimidating tone with a preschool child in his/her class as the children awaited their turn to use the restroom. The staff stated in a threatening and frightening manner, “Sit Down!” with clinched teeth. The child care center director was also present during this observation and spoke with the staff out of concern for his/her behavior. Interview with the child care center director revealed there have been some concerns brought to her attention with regard to the staff and she has attempted to address those concerns through coaching.
