General: Based on Interview, and review of documentation, it was determined the child care center failed to assure three (3) children were provided with adequate supervision at all times by a qualified staff person who ensures the children are within scope of vision and range of voice. Interviews and review of written statements revealed the following information: On 6/1/16 at approximately 10:15 a.m., a group of twenty (20) three-year-old children were playing on the outdoor playground for at least fifteen (15) minutes, while two (2) staff members were engaged in conversation. When a third staff member returned from a fifteen (15) minute break, the staff members gathered the children to return to the building and realized three (3) children were missing from the group for an undetermined amount of time. The staff member returning from break checked the perimeter of the playground, noticed an opening in the back gate of the playground, due to the gate failing to be secured properly, seen the three (3) children in an adjacent field, which was not located on the childcare premises, and brought the children back to the group. Once the children were located, the amount of time took to retrieve the children was no more than one (1) minute; however, the children had already been discovered by two (2) community members. Interview with one (1) of the community members revealed he seen an unidentified female with three (3) children in a field and stopped to see if she needed assistance. He stated she informed him that she had discovered the children unattended and was in the process of notifying local law enforcement officials. Based on review of the classroom's daily schedule, from 10-11 a.m.the classroom was scheduled to be outside or in the multi-purpose room. The staff member did not follow the schedule and give the children any gross motor play time.
