The orange wall located in the pre-school classroom behind the sink and the green wall in the two-year-old classroom had several areas were the paint had chipped and was peeling. The windowsill in the pre-school classroom had a buildup of dust and debris. Observation of the ceiling in the hallway revealed a brown stain with the drywall cracked, which was indicative of a water stain. Upon surveyor’s arrival at the center at 10:45am, a four-year-old was seated in a high chair with no food or activity. At approximately 11:06am, the four-year-old remained in the high chair, but was given a toy to play with, while the two (2) toddlers in the room walked around and played with puzzles and other activities. At approximately 11:11am, lunch was served, and the four-year-old ate lunch in the high chair. At approximately 11:40am, the two (2) toddlers were laid down for rest-time, while the four-year-old remained in the high chair. The four-year-old continued to sit in the high chair, awake, and played with a toy or sang to herself during rest-time. At approximately 1:53pm, the four-year-old was finally removed from the high chair and taken outside to the playground with one of the toddlers who had finished resting. Consequently, the four-year-old was kept in a high chair from approximately 10:45am – 1:53pm. The owner/director acknowledged the four-year-old was kept in the high chair during this time. In the One-Year-Old Room, a CD player was placed on a high shelf and plugged into the wall, but the long cord was dangling very low, right above the table used by the children, in reach of children in care. Observation revealed only two (2) staff members and the owner/director were present and on duty for the duration of the survey. The owner/director revealed the two (2) staff members and herself are the only persons who work at the center. Observation revealed both staff members were needed to maintain minimum staff-to-child ratios while the owner/director prepared lunch. The staff schedule revealed: (A) The staff member hired 5/2/16 works 8:30am-6:00pm as the Toddler Teacher; (B) The staff member hired 10/10/16 works 8:30am- 6:00pm as the Preschool Teacher; and (C) The owner/director works 6:00am-6:00pm as the Director and Cook. Review of staff files revealed only three (3) personnel files, two (2) files for the aforementioned staff members and one (1) file for the owner/director, were presented for review at the time of survey. Consequently, the center failed to have two (2) qualified substitutes available in case of need. Observation of the Toddler classroom revealed that there were numerous outlets that were uncovered. Review of children's files revealed that a child with the enrollment date of 07/11/2014 was listed as a vegetarian by his parents in the enrollment form. Observation of lunch revealed that the child with the enrollment date of 07/11/2014 was served chicken nuggets. The DRCC surveyor notified the staff that the previously mentioned child was a vegetarian and the Director prepared a vegetarian replacement. Observation of the kitchen revealed that a staff member turned on the electric skillet and began to heat oil. Continued observation of the kitchen revealed that the staff member that turned on the electric skillet walked back to the classroom, leaving the hot oil unattended. Observation of lunch revealed that two (2) infants were placed on mats due to falling asleep in high chairs prior to eating. Continued observation of the two (2) infants sleeping on mats on the floor in the middle of the lunch room revealed that a staff member was continuously stepping over the infants to serve food and food was being dropped on the infants as the other children ate lunch. Observation of the Preschool Classroom revealed that there were two children that were physically fighting for at least a thirty (30) seconds without staff intervention in a classroom where an 18 month old child was present.
