Review of the staff member's Authorization for Employment document revealed the staff member had lived in Florida until 2021. Review of the staff member’s record revealed he/she obtained a Colorado driver’s license. Interview with the staff member stated that he/she had lived in the state of Kentucky less than one (1) year. 4) A record for one (1) staff member (DOH: 07/31/23) failed to contain a completed out of state background check from Virginia, including a Virginia sex offender registry, Virginia criminal background check, and a Virginia child abuse and neglect check on the day of the inspection. Review of the staff member’s record revealed the staff member obtained a Virginia driver’s license. Interview with the staff in charge revealed that the staff member moved to Kentucky on 08/07/2022. Therefore, the staff member has not resided in Kentucky for five (5) years or longer. Review of the (KARES) database verified this information. Interview with multiple sources revealed that this occurred due to failure to read the ingredient content label on the food that was provided to the child, on the part of the staff. Additionally, the sources reported that an EpiPen had to be administered to the child as a result of the allergic reaction.
