General: Based on observation, the outdoor play area was not free of foreseeable hazards, evidenced by the following observations: 1. The grass and weeds were overgrown in several areas; 2. The handrail next to the steps by the back door was rusting and contained peeling paint; 3. The fence in the back of the yard was damaged by a portion of a tree on the fence, resulting in the fence being severely bent and no longer creating an adequate barrier to the next yard and its contents; and 4. The basement window was broken and covered with wood; however, a large piece of broken glass remained in the window and was accessible from the outdoor play area.
