General: Based on observation, ceiling contained brown and yellow circular stains, indicative of water damage in the following classrooms: Toddlers 1 and School Age. 1. The floor in the School Age classroom did not fit tightly, leaving a gap of approximately 1 inch between the wall and the floor. 3. The floor tiles were missing on the inside playground, exposing grimy concrete. A round hole approximately one inch in diameter was present on the wall next to the door in the inside playground, creating a significant opening. Paint on the inside playground wall was peeling, exposing concrete, and there was a dry, yellow substance of unknown origin present. The floor at the entrance to the inside playground was covered with a plywood sheet heavily soiled with a dark, unknown substance.
