Based on interviews and observation, the center failed to assure that heaters were protected. Multiple space heaters were utilized throughout the center. Some were placed on the floor, while others were on shelves. Those on shelves remained accessible to children due to the location of the heaters, the height of the shelves, or the electrical cords hanging low from the shelves. Two (2) heaters were placed on the floor with two (2) tables used as barricades around the heaters. Although the tables deterred access to the heaters, they did not fully protect the heaters from being accessible to children.
