Observation revealed the staff-in-charge hired 4/2/18 was present for the duration of the survey. Review of staff files revealed the staff-in-charge’s hire date was clearly documented as 4/2/18 on multiple documents. The staff-in-charge and owner/director acknowledged the hire date was listed as 4/2/18 in the staff file. Review of the KARES system revealed the aforementioned staff member’s name was not listed on the records for this center. The owner/director and staff-in-charge revealed the background checks were completed; however, they could not locate this documentation at the time of survey. Observation revealed the Infant Room and the One-and-Two-Year-Old Room were adjacent to one another, and the door connecting the rooms remained open during the survey. There was one (1) infant in the Infant Room and three (3) one-year-olds in the One-and-Two-Year-Old Room. For a large part of the survey, the sole caregiver in the Infant Room and the sole caregiver in the One-and-Two-Year-Old Room had visibility of all four (4) of the aforementioned children by way of the open door; however, at 11:19a.m., the staff member in the Infant Room walked to areas in the Infant Room which were out of the scope of vision of the children in the One-and-Two-Year-Old Room; thereby, leaving the staff member in the One-and-Two-Year-Old Room with sole supervisory authority over three (3) one-year-olds for an undetermined amount of time.
