General: Based on observation, review of documentation, and interview, this regulatory requirement was not met. The surveyor observed one (1) staff member supervising twenty- four (24) school age children alone in the cafeteria for approximately seven (7) minutes until a second staff member joined the group. A review of staff files revealed that the staff member working alone with the children was seventeen (17) year old. The area director arrived at the end of the inspection and stated that two (2) qualified staff members are typically present in the cafeteria with the underage staff member; however, one (1) staff member quit earlier in the day. General: Based on observation, this regulatory requirement was not met. The surveyor entered the cafeteria, which is the main childcare space for this program, at 4:15 pm and observed one (1) staff member present who was preparing the room for the children. Children started entering the cafeteria approximately one (1) minute after the surveyor arrived. By 4:18 pm twenty-two (22) children were present with the one (1) staff member and by 4:21 pm twenty-four (24) children were present. The youngest child was identified as a five- year-old; therefore, the minimum staff to child ratio would need to be one staff person for fifteen (15) children. A second staff member joined the group at 4:25 pm. Therefore, staff-to- child ratios were not in compliance for seven (7) minutes. It is important to note that the staff member alone with the children was identified as a seventeen-year-old high school student.
